OFFICIAL CONTEST RULES, TERMS AND CONDITIONS

By submitting this required entry package which includes the above Personal Information Form which will be submitted electronically by pressing this forms submit button, plus mailing a non refundable entry fee payable to: Studio 12 Huntsville, and mailed to: P.O. Box 554 Huntsville, TX 77342, I agree to be bound by the following.

I understand that music and performance appreciation is subjective and that all decisions based upon my performance will be made objectively and without any prejudice or personal bias.

I understand that I must pay a $25.00 entry fee, until June 1, 2012 whereas the entry fee increases to $35.00. I understand that this fee has been put in place to pay for the processing of my entry package only, and not as a purchase of placement within the show, and as such, it will not be refunded under any circumstance, including non acceptance into the show. I also understand that there is a final deadline for entry submission, and that any entry packages received that are post marked after June 15 will be subject to NOT BE INCLUDED FOR CONSIDERATION AS A CONTESTANT ENTRY - WITH NO REFUND GIVEN.

I understand that after a complete review of my entry package, including a full review of my CD or mp3 audition, the judges’ final decision might not be one that favors my performance high enough to be placed within the initial live featured show roster. To this end I understand that there is no appeal, and that the judges’ decision is final.

I further understand that these same rules concerning the overall judgment of my performance apply if I progress throughout the entirety of the competition.

I agree to all the performance rules as stated under "Performance Rules" on the contest information page of this website. I further understand that some of the basic performance rules (song length, order of songs, order of performance, Etc.) may vary or even be modified so as to accommodate the flow of the show as the series progresses.

I further understand that if I'm selected to participate, that I will be added to the live feature show portion of the roster, and until eliminated, I must be available to fully participate in all of the live shows which will be held Saturday Jun 23rd, 30th, and Saturday July 14th, 2012.

I, THEREFORE, for good and valuable consideration, receipt of which is hereby acknowledged, being of legal age or by my parent or guardians’ consent – as acknowledged by their initials in the above form, hereby give Huntsville Superstar, and it’s licensees, successors, legal representatives, and assignees the absolute and irrevocable right and permission to use my name and likeness, and to use, reproduce, edit, exhibit, project, display, copyright, publish and/or resell photographic pictures and/or moving images and/or aural recordings of me with or without my voice, or in which I may be included in whole or part, photographed, taped, videotaped, and/or recorded throughout the entire duration of the 2012 Huntsville Superstar series held during the months of April through July in the year of 2012, and thereafter, and to circulate the same in all forms and media for art, advertising, training, trade, competition of every description and/or any other lawful purpose whatsoever. I also consent to the use of any printed matter in conjunction therewith.

This release also applies as nonfiction to productions of motion picture video or videotape and radio broadcast, including industrial productions and documentary productions. I further give Huntsville Superstar the right to reproduce in any manner whatsoever any recordings made by Huntsville Superstar my voice and all instrumental, musical or other sound effects produced by myself.

I hereby waive any right that I may have to inspect and/or approve the finished product or products or the editorial, advertising, or printed copy or sound track that may be used in connection therewith and any right that I may have to control the use to which said product, products, copy and/or soundtrack may be applied.

Further, I am not an employee of Huntsville Superstar and instead represent myself as an independent contractor, acknowledging that I am solely responsible for the reporting and payment of any and all taxes which may arise from any monetary payment or winnings resulting from this agreement.

I hereby release, discharge, and agree to save harmless Huntsville Superstar, their licensees, successors, legal representatives, and assignees from any tax liability which may result from this agreement.

I hereby warrant that I am of full age and have every right to contract in my own name in the above regard. I state further that I have read the above authorization, release, and agreement, prior to its execution, and that I am fully familiar with the contents thereof.

I understand that this Release shall not expire. I further represent that I am over the age of eighteen years, and possess full contractual rights to enter into this release, or that my parent/legal guardian has read and gives consent as indicated in the above form, and, who by their consent as indicated above also affirm they have legal authority to bind me (a minor), and consent to the contract as outlined.

I hereby certify and represent that I or my parent/legal guardian have read the foregoing and fully understand the meaning and effect thereof and, intending to be legally bound, here hereunto set as I submit this electronic form and agreement.

I understand that I will be responsible for any copyright infringement or subsequent licensing fees imposed on behalf of the songs that I perform throughout the contest.
